Description:

Care Coordination provides high fidelity "Wraparound" through the use of the Child and Family Team process. The Wraparound process and the written Plan of Care it develops are designed to be culturally competent, strengths based and organized around family members’ own perceptions of their needs, goals, and vision. The primary goal of Care Coordination is to support families to support themselves by finding lasting ways to meet their needs and maintain their children safely at home.
